This is our new writing rubric that is used for grades three through eight to assess writing. Grades one and two will be using similar language in preparing kids for writing using these six traits.
Authorization Agreement For Direct Payment of tuition.
This form allows St. Luke to direct debit your account for tuition payments.
Each student is asked to have the supplies listed for his/her grade level on the first day of school. Please put child’s name on all supplies and clothing. Art Classes will meet on the first day of school.
Field Trip Permission and Activity Release Form.
Note that this form must be completed and returned to school before any child may leave the facility for an activity. Verbal permission is not acceptable.
