Registration for St. Luke Athletic teams must be submitted by the appropriate deadline for the sport. There is a late fee of $10 if registrations are received after 5 PM on the date due. Registrations received at evaluations or later are not guaranteed a spot on our St. Luke teams; however, every effort will be made to accommodate these children if there are available spots on our teams.

This is the form used to register for any CYO sports at St. Luke Catholic Church. Please fill out the form completely and return to the Athletic Director with the appropriate fee. Individual sports fees are listed on the registration form. (Please do not use one form to register more than two (2) children.)
